Report: Crew integrity a factor in Md. floor collapse

ROCKVILLE, Md. — Miscommunication and crew integrity were among factors contributing to serious injuries suffered by a firefighter who fell through a floor and into a basement, according to an investigation report.

Firefighter Joel Rogozinski, of the Rockville Volunteer Fire Department in Maryland, was performing search operations in an area where the fire had just been knocked down when the accident happened.

He managed to escape through a basement window, but had to be treated for significant injuries at the MedStar Burn Unit of the Washington Hospital Center.

The Significant Injury Investigation Report was released by the Montgomery County Fire and Rescue Service and was presented to those involved in the incident earlier this week.

The report altogether makes 47 recommendations on contributing factors including fireground operations, risk vs. benefit analysis, accountability, mayday, communications, personnel training and certification, PPE/SCBA and other equipment.
